CLAIMS PROCEDURES


SETTLEMENT OF CLAIM CLAUSE

The amount of any loss for which we may be liable under this Policy shall be paid within thirty (30) days after proof of loss is received by us and ascertainment of the loss is made either by agreement between the Insured and the Company or by arbitration; but if such ascertainment is not had or made within sixty (60) days after such receipt by the Company of proof of loss, then the loss shall be paid within ninety (90) days after such receipt.

Important Notice to the Aksitext Certificate Holder

In the unexpected event of your death due to accident, Philam Life will provide your family an insurance benefit of ten thousand pesos (Php 10,000).

FREQUENTLY ASKED QUESTIONS ON CLAIMS:

1. Who can be my designated beneficiary or who can claim my insurance?

For claims, we are guided by the hierarchy of beneficiaries mandated both by the Philippine Insurance Commission and the International Insurance Code. These are as follows:

  1. legal widow or widower
  2. surviving children (legitimate or illegitimate with necessary documents establishing that you are his/her parent)
  3. surviving parents
  4. surviving brothers and sisters
  5. executors or administrators of estate

2. Where will my beneficiary go to claim the insurance?

Your beneficiary can go to any Philam Life office to file the claim. Documents required are:

  1. original or certified true copy of the police report
  2. barangay certification of time and date of accident/death
  3. claimant's form (available at all Philam Life offices/branches)
  4. original or certified true copy of death certificate
  5. proof of relationship to the insured (e.g. marriage license if spouse, birth certificate if child, etc.)
Additionally, your beneficiary must be at least 18 years of age to claim. If below 18, child must be accompanied by legal guardian and an Affidavit of Legal Guardianship should be presented.

3. How long can my beneficiary get the benefit?

The benefit shall be paid within thirty (30) days after necessary documents have been received and verified. If such verification is not made within sixty (60) days after such receipt of documents, then the benefit shall be paid within ninety (90) days after receipt

4. Will the insurance proceeds be in cash or check?

Insurance proceeds will be in the form of check payable to the beneficiary. This can immediately be encashed at the branch's depository bank subject to the bank's identification process.

5. Does it mean that an accidental death has to occur in the Philippines to claim the benefit?

No, your beneficiary will still be entitled to the P10,000 insurance even if the death caused by accident happened outside the Philippines. There are a few countries excluded from this rule. The coverage is for accidental death only. Death due to illness does not qualify one to claim. Benefits will not be given for injury or events caused directly or indirectly, wholly or partly:

  1. by intentionally self-inflicted injury, suicide or any attempt thereat , while sane or insane
  2. by war, invasion, act of foreign enemy, acts of terrorism, hostilities or warlike operations (whether war be declared or not), mutiny, riot, civil commotion, strike, civil war, rebellion, revolution, insurrections, conspiracy, military or usurped power, martial law or state of siege, or any of the events or causes which determine the proclamation or maintenance of martial law or state of siege, seizure, quarantine, or nationalization by or under the order of any government or public or local authority
  3. by any weapon or instrument employing atomic fission, thermonuclear fusion or any form of radiation, whether in time of peace or war
  4. by homicide or any attempt thereof, or physical injuries, occasioned by the provocation of the policyowner
