Suffering from an illness or an accident is an emotionally upsetting experience. Having to undergo hospital confinement or disability makes you worry from the uncertainties it bring. At these unforeseen events in your life, it is always comforting to know that you and your loved ones have your Philam Life insurance policy to provide you the assistance you truly need in these difficult times.

Accident

With this rider attached to your policy, it provides benefits on injuries resulting from an accident which may be payable even without a hospital confinement.

This benefit refers to the Accident & Health rider, Personal Accident rider (PA), Comprehensive Accident Indemnity Rider (CAIR) or the Medical Reimbursement (MR) rider attached to your policy. Please refer to page 3 of your life insurance policy contract for your schedule of benefits and specific details.

To file for an accident claim, following are the requirements:

• Duly accomplished claim forms:
  1. Claimant's Statement of Accident
  2. Certificate of Attending Physician
• Original or Certified Copy of the Statement of Account from the Hospital
  
  In the absence of the SOA, you may submit a Hospital Certification which should contain the inclusive confinement dates. This certification must be signed by an authorized personnel from the hospital's Billing or Records Section and NOT by the Attending Physician.
  
  
• Official Receipts of Hospital Bills, if hospitalized
• Police Report regarding the accident. If unavailable, insured's narration about the incident or statements of witnesses
• Photocopy of the Driver's License, if insured is the driver involving a vehicular accident

The processing of the claim begins when the basic claim forms and other related requirements are submitted to any Philam Life Office nationwide. Additional requirements may also be required as the company deem necessary in some instances.

Upon receipt of the required documentations, we will then begin the benefit assessment, evaluate the claim and provide you with commitment dates. You shall be notified of the final decision once available. If the claim is approved, proceeds may be released to the Philam Life office nearest you upon presentation of proper identification.
