Death Claim


Facing the loss of a loved one is tough. It is an inevitable event that sadly we will all go through. Your loved one has prepared for it through a Philam Life insurance policy which will provide you the assistance you need for a secure future.

Who Can File A Death Claim

The following may file for a death claim:

  • Designated Beneficiary. There are instances when the insured designated a set of primary and contingent beneficiary. If the primary beneficiary is alive, the proceeds will be given to him or be divided equally among them in case there are multiple primary beneficiaries. Contingent beneficiary will only be entitled to the proceeds when the primary beneficiaries pre-deceased the insured and the insured/owner was not able to update the beneficiary designations. In this case, a certified copy of the death certificate should be furnished.

    If the beneficiary is the spouse, Marriage Certificate must be provided.

    Any discrepancies should be accompanied by an Affidavit of Identity executed by two disinterested persons and duly notarized to attest that the discrepancies refer to one and the same person only.

  • Minor Beneficiary. If the appointed beneficiary happens to be a minor (below 18 years of age), the legally appointed guardian must complete the Claim form together with the minor's birth certificate.

    If the share of the minor is P50,000 and below , the guardian must file an Affidavit of Legal Guardianship.

    If the share of the minor is greater than P50,000 , a Judicial Bond must be executed by the surviving parent and must be filed at the Regional Trial Court (RTC) nearest the place of residence of the minor, in compliance with the Philippine Law to protect the interest of the minor.

    If the beneficiary is an Orphan , the guardian must file for guardianship of the minor with the Regional Trial Court. This requirement is composed of the Court Order, Oath of Office and the Decision necessary to protect the interests of orphaned beneficiary.

  • Assignee. When the policy is assigned, the Claim form must be accomplished by the assignee. If the policy assignment is no longer effective, a Release of Assignment from the Assignee must be submitted.
