Death Involving Accident


Facing the loss of a loved one is tough. It is an inevitable event that sadly we will all go through. Your loved one has prepared for it through a Philam Life insurance policy which will provide you the assistance you need for a secure future.

Death by Accident or Unnatural Means

With this rider attached to your policy, it provides additional death benefits if death is caused by accident or violent means, subject to the provisions of the policy contract.

This benefit refers to the Accidental Death Benefit (ADB) rider, Comprehensive Accident Indemnity Rider (CAIR), or the Accident and Health (A&H) policy/rider. Please refer to page 3 of your life insurance policy contract for your schedule of benefits and specific details.

To file for a death claim, following are the requirements:

  • Duly accomplished claim forms:
    1. Certificate of Claimant
    2. Certificate of Attending Physician
    3. Certificate of Identifying Witness
  • Death Certificate, duly certified by the Office of the Local Civil Registrar
  • Birth Certificate of the insured or payor
  • Birth Certificate of minor beneficiaries
  • Judicial Bond for minor beneficiaries if proceeds are more than P50,000 each

    If a beneficiary is a minor, and whose share of the proceeds is greater than P50,000.00, a Petition for the approval of the bond shall be filed in the proper court where the minor resides.
    In case other circumstances are present, the applicable laws found in the Civil Code of the Philippines shall apply.


  • Marriage Contract, if the spouse is a beneficiary
  • Philam Life Insurance Contract. If unavailable, submit affidavit of loss
  • Police Report and/or statements of witnesses
  • Certificate of Postmortem Examination or Autopsy Report, if available

The processing of the claim begins when the beneficiary submits the basic claim forms and other related requirements to any Philam Life Office nationwide. Additional requirements may also be required as the company deem necessary in some instances such as discrepancies on the deceased insured's or the beneficiary's identity or related reports for accident cases.

Upon receipt of the required documentations, we will then begin the benefit assessment, evaluate the claim and provide you with commitment dates. You shall be notified of the final decision once available. If the claim is approved, proceeds may be released to the Philam Life office nearest you upon presentation of proper identification.
