At these unforeseen events in your life, it is always comforting to know that you and your loved ones have your Philam Life insurance policy to provide you the assistance you truly need in these difficult times.

Disability

Initial Filing of Disability Claim

Disability benefits excuses the payment of all premium due under the life insurance policy when the insured or the payor has total and permanent disability, subject to the provisions of the policy contract.

This benefit refers to the Waiver of Premium (WP) rider or the Payor's Benefit (PB) rider attached to your policy. Please refer to page 3 of your life insurance policy contract for your schedule of benefits and specific details.

To file for an initial disability claim, following are the requirements:

  • Duly accomplished claim forms:
    1. Claimant's Statement of Total Disability
    2. Attending Physician's Statement
  • Hospital Records, if insured was hospitalized
  • If available, documents showing approval of disability claim from other companies such as from SSS, GSIS, or other life insurance companies.


Continuance of Disability Claim

Disability benefits are being reviewed regularly, and per policy contract provisions, the Company has the right to ask for additional proofs of disability to examine the disabled insured or payor.

To file for a continuance of disability claim, following are the requirements:

  • Duly accomplished claim forms:
    1. Claimant's Statement regarding Continuance of Total Disability
    2. Attending Physician's Statement regarding Continuance of Total Disability
  • If available, documents showing approval of disability claim from other companies such as from SSS, GSIS, or other life insurance companies.

The processing of the claim begins when the basic claim forms and other related requirements are submitted to any Philam Life Office nationwide. Additional requirements may also be required as the company deem necessary in some instances.

Upon receipt of the required documentations, we will then begin the benefit assessment, evaluate the claim and provide you with commitment dates. You shall be notified of the final decision once available. If the claim is approved, proceeds may be released to the Philam Life office nearest you upon presentation of proper identification.
